Planning your child’s first visit to SCIP? On this page, you’ll find information you need to get started. To save you time, we’ve also included our new patient forms, which you can print out and complete prior to visiting our office.
The Initial Visit
Our pediatricians and staff do whatever it takes to make every visit to our office a pleasant, relaxed experience. With all of the important information about our practice available on our website, you can feel confident that you and child are well-prepared for your first appointment. We look forward to meeting you.
If you’re preparing for your child’s first visit, you can do a few things to help expedite the appointment. Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
Patient Forms
We are happy to provide our intake office forms for convenient download on your home computer. These forms can be completed on your computer and emailed to scip.miracosta@gmail.com or printed and brought in with you at the first appointment. Each intake packet contains all the forms you will need to fill out for your appointment. If you have any questions or need individual copies of any form, please call our office and our courteous staff will assist you in preparing for your child’s first visit.
Please note: For families with multiple children only page 1-2 need to be unique for each child, the other pages can include all children anywhere it is indicated to include patient name or date of birth.
